## When to choose which

Choose ProTaper Ultimate when

- you want more of the canal wall touched: Ultimate to F2 left 17.1 % of mandibular molar walls unprepared against 22.5 % for WaveOne Gold Primary (PMID 41964818)
- torsional strength at the ISO test point matters more to you than bench fatigue life: Ultimate F2 broke at 1.45 Ncm against 1.10 Ncm for the Primary (PMID 41964818)

Choose WaveOne Gold when

- the canal is curved and bench fatigue life is the concern: the Primary reached 1,883.10 cycles at 35 °C against 726.70 for Ultimate F2 (PMID 41964818)
- you want one file per canal with shaping equal to Ultimate in curved mesiobuccal canals: transportation and centring did not differ at 3, 5 or 7 mm in 25 to 40 degree maxillary molar roots (PMID 41685849)
- you pair the file with sonic activation and debris extrusion worries you: the WaveOne Gold plus sonic combination extruded the least debris of nine combinations tested (PMID 41094147)

## What the comparative evidence shows

3 comparative full-text studies read for this pair

Three comparative full texts were read for this pair: one micro-CT shaping study in maxillary molars, one debris-extrusion study and one combined bench and micro-CT series in mandibular molars. Ultimate is a multi-file rotary system with an off-centred parallelogram cross-section and file-specific heat treatments; WaveOne Gold Primary is a single reciprocating file with a parallelogram cross-section and a 7 % apical taper. Three further comparative papers, on pericervical dentin and smear layer, on severely curved canals and on rotary against manual use, are paywalled and were not read, so the read set is thinner than for the other ProTaper pairs.

On shaping, 42 mesiobuccal canals of maxillary molars with 25 to 40 degrees of curvature showed no difference in transportation or centring between WaveOne Gold, Ultimate and TruNatomy at 3, 5 or 7 mm, and no canal was displaced more than 0.2 mm (PMID 41685849). In 50 mandibular first molars, Ultimate to F2 and the Primary produced the same volume increase, but the Primary left more of the wall untouched (22.5 % against 17.1 %) and Ultimate showed the highest transportation of five systems at the 6 mm level of the mesial canals (PMID 41964818).

On mechanics, the same series found the Primary far ahead on cyclic fatigue at 35 °C in a 60-degree steel canal (1,883.10 cycles against 726.70), Ultimate F2 ahead on maximum torque (1.45 Ncm against 1.10 Ncm) and on angular deflection (398.87 degrees against 310.87), and Ultimate F2 the stiffest of the five files in a 60-degree bending test (PMID 41964818). The authors present these as whole-system contrasts and warn that taper, cross-section and kinematics are confounded.

On debris, 135 straight mandibular premolars prepared at 35 °C showed no difference in apical extrusion between Ultimate to F3, WaveOne Gold to Medium and a third rotary system, while passive ultrasonic activation extruded more than sonic or conventional irrigation whichever file was used; the WaveOne Gold plus sonic combination extruded the least (PMID 41094147).

Nothing in the read set compares the two files on postoperative pain, healing, dentinal defects or retreatment.

In the one bench comparison read for this page, WaveOne Gold Primary survived about two and a half times as many cycles as Ultimate F2 at 35 °C in a 60-degree canal (PMID 41964818).
