## Protocol

### Glide Path

1. **Create Manual Glide Path** — Check canal patency and create a glide path to the apical constriction or foramen using C-PILOT files (ISO 06, 08, 10). Determine working length with an electronic apex locator.
   - Pre-curve hand files for curved canals
   - Establish patency to at least ISO #10 before using rotary files
   - Use chelating agent (EDTA) to facilitate progression
   - ⚠ Do not probe the pulp chamber floor with the Mtwo 10/.04
   - ⚠ Never force hand files apically
2. **Mtwo 10/.04 to Working Length** — Insert the rotating 10/.04 into the canal without touching the walls. Allow it to advance automatically using brushing movements. If resistance is felt, pull back 1-2 mm and use lateral brushing strokes on the canal walls to create space, then allow the file to progress again.    - No apical pressure — let the instrument advance automatically
   - Clean flutes in the interim stand between passes
   - Use FileCare EDTA to facilitate progression
   - ⚠ Do not use in a picking movement
   - ⚠ Do not force the instrument to reach working length
   - ⚠ Bring the instrument to working length only once, then proceed to the next

### Shaping

1. **Mtwo 15/.05 and 20/.06 to Working Length** — Continue with 15/.05 then 20/.06, each to full working length using the same Simultaneous Shaping technique. Each instrument creates a glide path for the next. Irrigate between instruments. _(File: 15/.05, 20/.06 · 280 RPM · 1.3 / 2.1 Ncm · Simultaneous Shaping (brushing))_
   - If larger canals allow hand file #15 or #20 to WL easily, start Mtwo sequence at the corresponding size
   - Irrigate the root canal between each instrument change
   - Each Mtwo instrument enlarges the canal entrance progressively — no separate coronal flaring needed
   - ⚠ Monitor torque alarm signals — frequent alarms indicate excessive stress
   - ⚠ If instrument stalls, pull back and create space with brushing before re-advancing
2. **Mtwo 25/.06 to Working Length (Basic Sequence Completion)** — Complete the basic sequence with 25/.06 to full working length. This is the standard final preparation size for most canals. Gauge the foramen with an ISO #30 hand file: if snug at 1 mm short of WL, preparation is complete. _(File: 25/.06 · 280 RPM · 2.3 Ncm · Simultaneous Shaping (brushing))_
   - Apical gauging after 25/.06: insert ISO #30 hand file at WL
   - If hand file #30 is snug at WL minus 1 mm — canal is ready for obturation
   - If hand file #30 reaches full WL — further apical enlargement is needed
   - ⚠ Do not skip apical gauging — it determines if further shaping is needed

### Finishing

1. **Further Apical Enlargement (If Needed)** — If apical gauging indicates further enlargement is needed, continue with 30/.05, then 35/.04 if necessary, gauging with a hand file after each. For increased coronal taper, 25/.07, 30/.06, 35/.06, or 40/.06 are available.    - Reduced-taper files (30/.05, 35/.04, 40/.04) enlarge the apex without weakening the tooth coronally
   - Larger-taper files (25/.07, 30/.06, 35/.06, 40/.06) are available when more flare is desired
   - Verify preparation with wet vs dry debris test: red/wet debris = enlarge further; white/dry chips = target size reached
   - ⚠ Do not over-enlarge in thin roots
   - ⚠ Instruments with larger ISO sizes have a reduced cross-section for maintained flexibility
2. **Obturation** — Irrigate thoroughly with NaOCl and EDTA. Dry with matching Mtwo paper points. Obturate with Mtwo gutta-percha points (cold lateral) or GuttaFusion obturators (warm technique).
   - Mtwo matching gutta-percha and paper points available for all preparation sizes
   - GuttaFusion obturators (sizes 20-55) recommended for warm three-dimensional filling
   - Use GuttaFusion Size Verifiers before placing obturators

## Clinical Tips

- All files use 280 RPM (recommended; range 250-350 RPM per IFU). Torque is size-specific: 1.2 Ncm (10/.04), 1.3 Ncm (15/.05), 2.1 Ncm (20/.06), 2.3 Ncm (25/.06). VDW motors have pre-programmed Mtwo settings.
- Simultaneous Shaping means each file goes to full working length — no crown-down staging. This preserves tooth substance compared to coronal-first techniques.
- Brushing file movement: when resistance is felt, pull back 1-2 mm and stroke laterally against the canal wall (like a Hedstroem file), then allow the instrument to re-advance apically.
- No separate coronal flaring is needed — Mtwo's lateral cutting efficiency gradually widens the canal entrance with each file used.
- Apical gauging with ISO #30 hand file after the basic sequence determines if further enlargement is needed.
- Retreatment files R15/.05 (narrow canals, 0.3 Ncm) and R25/.05 (medium/wide canals, 1.2 Ncm) have active cutting tips for gutta-percha removal.
- Compatible with VDW.GOLD RECIPROC, VDW.SILVER RECIPROC, and VDW.CONNECT Drive motors (pre-programmed Mtwo settings available).

## Warnings

- Reusable sterile instrument per 2023 IFU — cleaned undamaged instruments validated for up to 8 processing cycles; silicone stops are single-use and must be replaced after each use
- VDW states it is safest to use Mtwo instruments only once — if reusing, document frequency with control stickers and inspect carefully
- Contains nickel — do not use on patients with known nickel allergy
